The New Intimacy: How AI Is Rewiring Our Minds

The machines did not arrive as monsters. They arrived as helpers—polite, ever-awake, and eager to please. In offices and bedrooms, in clinics and classrooms, people now ask chatbots to plan a week, critique a draft, calm a panic, even say goodnight. The question is no longer whether these systems can help. It is what they are doing, quietly and cumulatively, to how we think, feel, and attach.

Therapy at the speed of text

In March, a randomized trial published in NEJM AI reported that an expert-tuned therapy chatbot produced clinically significant reductions in depression, anxiety, and eating-disorder symptoms over four weeks. The design was conservative—guardrails, clinical aims, measured outcomes—and the results were not subtle. For a certain slice of need, under supervision, the machine helped. 

Public-health bodies have taken note—but drawn a sharp boundary. The World Health Organization’s guidance on large multimodal models urges validation, transparency, and clinical oversight when models enter health contexts. General chatbots, it warns, are not therapy. They are tools that must be proven and supervised. 

Britain’s NHS has moved further, advising young people not to use open chatbots as substitutes for treatment, citing risks of harmful advice and the tendency of engagement-driven systems to mirror distress rather than challenge it. It is a blunt message in an age of soft lines: in youth mental health, safety beats novelty. 

The child-safety turn

American regulators are now training their sights on chatbots’ effects on minors. The Federal Trade Commission is preparing to demand internal documents from major firms as part of a broad inquiry into psychological harms and design choices. Whatever the findings, the signal is clear: the burden of proof is shifting toward age-gating, parental controls, escalation pathways, and audit logs. 

Offloading the mind

Every new tool changes what we keep in our heads. A 2025 study in Societies links heavier AI use to cognitive offloading and, in turn, lower critical-thinking scores—a mediation pattern that will ring familiar to anyone who has watched calculators reshape algebra or GPS dissolve a sense of streets. The mechanism is not mystical: when an assistant makes reasoning feel optional, we often let it be. 

The education research is layered rather than conclusive. Meta-analyses this year find benefits when students use generative AI to construct and elaborate knowledge—but costs when they lean on it for answers and form. In other words, the pedagogy matters: write first, then consult; ask the model to critique, not to compose. 

The new companions

A second frontier is attachment. As chatbots gain memory, mimicry, and a feel for our rhythms, relationships form. New work out of Penn’s Annenberg School maps how users describe AI “partners”—comforting, consistent, sometimes preferable to human messiness. Clinicians warn that what feels safe can also become sticky; for adolescents especially, it risks displacing friction-filled relationships that teach resilience. 

Stanford psychiatrists put it plainly: “friend-mode” chatbots should not be used by children and teens. The design patterns that make bots feel endlessly patient—the flattery, the availability, the non-judgment—also deepen dependency. The line between soothing and substituting is thin. 

When the helper becomes the anchor

There is a quieter effect that plays out in boardrooms and clinics: anchoring. Early suggestions from a model can become the gravity well around which subsequent judgments orbit. A growing literature documents this in decision support—including clinical contexts—where sequence and framing subtly steer outcomes. Calibrating for this means changing workflows, not just prompts. 

Knowledge-work studies suggest a broader psychological paradox. AI boosts speed and flattens variance—especially for lower-experience workers—yet can erode effortful thinking if over-trusted. Organizations that get net gains do two things well: they force justification (why accept or reject the model’s suggestion?) and they separate drafting from deciding. 

Edge cases, headlines, and the fog

What about the stories of “AI psychosis”—people sliding into delusion through late-night sessions with a model that never blinks? The most serious attempt to quantify this, by Astral Codex Ten, reads as a careful shrug: there are suggestive anecdotes, but the base rate looks low and the definitions slippery. Still, the combination of 24/7 availability and emotional mimicry deserves watchfulness, not hand-waving. 

How to think with a machine (without losing the plot)

Three rules emerge from the strongest evidence.

  1. Do not substitute general chatbots for care—especially for minors or high-risk users. If a conversation turns to self-harm, eating disorders, or acute distress, the correct next step is a human clinician, not a warmer prompt. (This is the one consensus across health guidance and regulators.)  
  2. Design for friction. Build guardrails that make offloading harder and scrutiny easier: write first, then ask the model to critique; demand rationales; schedule “AI-off” blocks; review anchors by requiring a second, independent pass. These are cheap interventions with disproportionate effect on thinking quality.  
  3. If you deploy at scale, deploy like a clinic. Age-gates, escalation paths, audit trails, and ongoing evaluation are not luxuries; they are the ticket to operate. The institutions that treat AI as a medical device—tested, monitored, documented—will keep both users and reputations intact.  

The mind will adapt; it always does. The danger is not that AI will think for us, but that we will stop noticing when it does. The opportunity is to decide—consciously, procedurally—what to keep in our heads and what to borrow from a machine. The line is not fixed. It is drawn, daily, by how we choose to use the tools that speak back.
